An "Advance directive" is an euphamism for "Do you want to be revived if they find you unconscious?"

Some people dont want to live as a vegetable, and VA likes it when you do an advance directive especially if its "DNR", which is "Do Not Ressucitate".  Very often old people in a nursing home dont want to be kept alive with tubes, if they are found not breathing they want to be allowed to die.

Thank you. That cleared up some doubts I had. I'm always questioning this type of VA behavior.
