VA Scandals
Shreddergate and other scandals
On the 13th of October, 2008, VA Watchdog dot Org broke the story about documents found in shredder bins at the VA's Detroit Regional Office.
A number of those documents were critical to veterans' disability claims.
As the story unfolded, VA finally began to release information on the incident and we learned that more than one Regional Office was involved in this shredder scandal.
The VA's Office of Inspector General (VAOIG) is investigating.
VA has instituted new guidelines regarding handling of documents.

This news is very startling, and I hope it isn’t too prevalent. https://www.kansascity.com/news/local/crime/article232443717.html In part: “The Kansas City Police Department is investigating the death of a veteran who had a fatal brain hemorrhage after a confrontation with Veterans Affairs Police last year. Department spokesman Jacob Becchina said the case of Dale Farhner has been assigned to Sgt. Richard Sharp, a longtime homicide detective. “The results of our investigation will ultimately be compiled and forwarded on to the prosecutor’s office for their decision as to what if any applicable charges may apply,” Becchina said. “There is little info that can be rele…

I am sure this may affect several vets on this site. https://www.military.com/daily-news/2019/06/07/inspector-general-finds-va-overcharged-disabled-vets-home-loans.html At the end of the article it states "The VA concurred with most of the IG recommendations and set a target date for identifying affected veterans and providing refunds by July 31."

(or survivors of a veteran) 700 who might get this letter.... https://www.disabledveterans.org/2019/06/03/several-veteran-deaths-linked-to-impaired-va-clinician/ In part: "On Friday, VA confirmed suspicions that several veteran deaths were linked to negligence by an impaired VA pathologist at Fayetteville VA Medical Center. The agency just announced the result of its 33,902 pathology results audit from 2005 to 2017. VA found 30 missed diagnosis posing serious risks to patients. The patients or family members were recently contacted. Fifteen of the veterans are now dead though it is not reported the extent to which the negligence played in each. …

https://www.disabledveterans.org/2019/04/30/tragic-veteran-suicide-at-cleveland-va-medical-center/ I am beginning to wonder what the common thread could possibly be- the VA has started hiring "Providers" who are not VA employees. They might well be completely qualified to treat veterans with MH issues ------but----are they----- Have any of these veterans been prescribed any medication that could cause or enhance suicidal ideation? Is you MH VA doctor a VA employee? Or contracted out by QTC, LHI, or VES?

Imagine the VA almost performing the wrong kind of surgery to resolve abdominal pain. After a couple of ultrasounds, a CT scan, and an MRI scan, the VA was eager and ready to dive into my urinary tract and perform surgery. The VA diagnosed me with a kidney stone too large to pass on its own. At the pre-surgical appointment, the VA-contracted urologist looked at the imaging and determined it is not a kidney stone. He showed me on the image and pointed to a the VA radiologist's note indicating I needed a CT scan "with contrast" to figure out what might be growing in there. VA doctor's missed it.

Ben Krause made an interesting observation: (but no surprise to me- when my husband was hired at the local VAMC,about 4-5 months later he was called into the Personnel office-wondering what he had done wrong, but the Personnel director thanked him because his background check was so good- but he told me it seemed odd to him that any federal background check would take so long -hecould have been a felon- all those months working at the VA - this was however in the 1980's- still I bet it happens these days as well--- even though a background check these days can be done by anyone on line if they are willing to pay for it- and they should go much faster too) …

In part: "The OIG team reviewed more than 5,000 of PCP-1’s patient encounters from October 1, 2015, through December 26, 2017. Of these, 1,370 were primary care encounters during which the patients had diagnoses that placed them at greater risk for adverse clinical outcomes (defined in this report as death, a change in the course of treatment or diagnosis, or a significant change in the patient’s level of care) if/when their BPs were not well controlled. The OIG’s review found that in 1,364 of the 1,370 encounters (99.5 percent), PCP-1 documented repeat BPs of 128/78." and "PCP-1 told the OIG team that documentation of the 128/78 BP readings was done to “t…

I was not surprised when I read about the shredded documents that was above this thread. Perhaps I will relay part of my story. At this point useless to detail all my disability claim problems, but suffice to say I was initially denied due to a false statement. (Please See, 18 U.S.C., Section 1001). What compounded this was that I had no idea this false statement was even asserted until I got my initial denial letter. Subsequently I received a letter from the VA telling me they were going to take away my co-payment exemption for the failure to file an updated means test. This is sort of where I got too smart for my own britches. It was mere inadvertence that I d…
