A veteran has the right to request an amendment to any information or records retrieved by the individual’s name or other individually-identifiable information contained in a VA system of records. This is specified in 38 CFR 1.579 and 45 CFR 164.526. The request must be in writing and mailed or delivered to the facility, addressed to the privacy officer.

There is no specified form in which to use.
