Maybe I asked this here before, if so, forgive me.

Can someone give me an understanding on section II block 14? Does that mean the date I became 'too disabled to work' from fulltime work? or any work at all?

The reason I ask is because block 12 and 13 clearly state fulltime employment and work.

The date SSDI recognized as my disability start date is more recent. The date I was unable to work fulltime was many years ago. I could write a statement explaining and add it to block 24.

Any ideas? Am updating 21-8940 for dro.Thank you~!

cg - I'd add an attached page, referencing block(s) 12 & 13 and giving a thorough expalnation. Be sure to put "see attachment or attached page" in those blocks. Be sure to put your name and claim number on that page, in case they become separated. jmo
