Question

After much deliberation decided to file for SC through the state dept. of vet affairs office after someone had the awareness to ask if I had ever gone to the mental health clinic while in service. I had repeatedly and was referred to a civilian psyche who prescribed medication, whose name I remembered and have found him still actively practicing and have an address. Did not put his current address on the 526 though, but have it available now.

I think you should also contact the doctor and get a copy of those records for yourself. Don't rely on "anyone" to do anything for you. Because you could end up disappointed

Bergie is right- and also try to get copies of all of your military records at the NARA web site and any other private treatment records.
