Release Of Information Form

Question

I am applying for an increase in my disability rating for my back. They sent me one consent to release information form. I have 4 different doctors offices I have records at. Does anyone know if I should put all 4 offices on that one form or should I have 4 forms one for each office for the VA to send them too.

Since the form is limited in space, you may want to use four separate forms and make sure you sign them and keep a copy of them all. You can also fill out one form and attach a letter with the other doctors' information; if you attach a separate letter make sure you sign, date and keep a copy of it also.

With almost all forms I fill them out and before I sign them I make a copy, then I sign the first one and send it in, that way if they lose the form, all you have to do is make another copy, then sign one and send it in, Saves a lot of frustration.
