Champ VA: http://www4.va.gov/vaforms/medical/pdf/vha-10-10d-fill.pdf MWR card or form 2765 This needs to go to the nearest installation that has an ID card issuing machine for DEERs. Regarding the ID cards, if you are not near a base, check this page and locate the nearest one by zip code. I recommend calling them first.
