Service dog application process

Question

I have decided to apply for a successor hearing dog. Things have changed since I received my 1st dog. The VA helped with reports and recommendation needed by the non-profit organization but the costs of obtaining were strictly mine. After I had my dog for a few years the VA started proving Veterinarian care, which was a godsend when he developed cancer. 

My question is what forms do I need to fill out for obtaining a service dog through prosthetics.

I tried to ask my doctor about it at my VA appointment and he told me they don't do anything. I need to go to a dog trainer and bring the letter to him and he will sign it. 

That link to the website above suggests the doctor will give me a referral and then prosthetics will start the process? 

I'm confused and can't find any specific information. Help please
