  1. These I have been using-- even has my va DRs listed STEVE & PAT http://www.ratemds.com/social/?q=node/11&country=0 http://www.vitals.com/doctors/#spec_state
  2. Sent my old shreading claims in SATURDAY---$!6.30 some cents NEXT DAY mail- return RECIEPT about last 26 itens on there 07 denial-- got 10% on left leg paraiisis (I didnt apply for) WHO KNEW? any way Ill see if they get the info and medical forms now. Steve & Pat
  3. dh=== My doctor wrote the consult up on a prescription form and they sent it internaly to fee basis- then they approve or whatever they want--- then contact you- mine took about 2 weeks but I went to fee basis and hurred them up== GOOD LUCK=== STEVE & PAT
  4. That will also go thru your provider "doctor or whoever handles your appointments at the VA - Such as I had my Doctor (nurse practicioner ) write me a CONSULT and they send it to fee basis to be approved. Like a prescription. If this CARE isnt available at your VA. I did a consult last year for my kidney disease (failure) == The consult got sent and I see the Nephroligist about once a month.Im going on my second year now. But your doctor or caregiver at the VA has to fill out the consult so you ca get FARMED OUT. They then send the billing information to whoever your doctor is and they can FAX the bill to fee basis. This pays whatever your doctor needs --with me the blood work and ETC. The hard part is convincing the doctor to fill out your request (consult.) But if the care isnt available at your VA you can request they supply it from the outside medical source. STEVE & PAT.
  5. HI-- The FEE BASIS is at your local area VAMC.HOSPITAL. Usally the fartherest building from the entrance as ours is. My card says; VA FEE BASIS OUTPATIENT MEDICALCARE AUTHORIZATION valid untill cancellrd by VA ======VA FORM 10-1174 MAR 2003 Then it has there information on the inside of the folded card. If I can get the kid (18 year old- YA MOM & DAD ARE 62 yo )to copy it to the forum later. Says to send bills monthly and ETC. Says wont pay bills after 2 years. If we need prescription they only pay 10 days worth. We photo copy the prescription and the pharmacy reciept to send to F.B. billing. ( and make copys for my file. you try to use your card often -or sometimes they say there isnt much need. BEEN THERE DONE THAT--- STEVE & PAT
  6. Fee Basis == I Have a yellow FEE BASIS CARD that will pay up to $125.00 MONTHLY for any reason -- EXCEPT as my card says EYEGLASSES and DENTAL=== You have to apply for it thru your local fee basis office== Mjne was because the nearest VA is more than 50 miles away =I had a card thru MPLS VA in Southern MN as now I have it out of ST CLOUD MN. Theyre very slow paying too. If anvthing over the $125.00 has to be autherized- If the VA cant do the needed tests STEVE & PAT
  7. I also have the travel pay office pay my OUTSOURCED visits to MY kidney specialist -my heart specialist and blood & urin tests at my local clinic. Some times he will call fee bacis to check. Im also alloted there meager $125.00 a month on any visits for as my card says-no rescrictions. You can get fee basis if you are too many miles from a VA facility at 100%. We have lived over 60 miles from our VAs. Also if your va makes your appointment at another facility they do so because they cant handle it at the closest va. HENCE travle should be paid to your appointment destination. I usually photocopy my appointment and the TRAVEL OFFICE staples it to my travel slip .If their not sure you have to wait for the check to be sent to YOU. Each kidney appointment consists of blood work & fluids and aweek later the appointment with my specialist. Last month I had 5 appts all together so NO withholding on #4 & 5 trips Hope this helps sombody. Steve & Pat in the STICKS N.Minnesota
  8. CORRECTION+++++++++++++++++++ ------------------------------------------------------------------------------------- I MEAN $1200.00 SAVED STEVE & PAT
  9. FEE BASIS___ USE THE I%RIS NUMBERS_ This what I used when the VA didnt want to pay there $18,000.00 bill to ST JOES=== THings were faster when IRIS started asking questions-- I very quickly got calls from the PATINT ADVOCATE_AND THE FEE BASIS office in St Cloud. Cant hurt to try - worked for me. Steve & Pat
  10. Well the day before yesterday the fee basis at our st. cloud va finally sent the LETTER saying they now are going to pay the hospital bill=imaging - urilogy doctor and the rest or ALL of the bill of $18,000 for my OCTOBER stay for emergency 7 day hospital stay. Even had the minnesota center for disease control concocting antibiotics for the infection. Fee basis turned it down- so I requested my file at FEE BASIS= NOTHING - SO I sent my nod==VISITED THEM{ I have one or two apps a month.AND ANOTHER VISIT, AND A DOCTORS letter= THE service officer sent a complaint- Next visit I gave the DRs letter--FOIA request- AND I used the IRIS 1=800 number IT worked - FEE BASIS AND THE PATIENT called within 20 minutes. So I found a use for the IRIS 1-800 number== Within a week ther reversal came- I also chewed them out for not paying the KIDNEY SPECIALISTS BILLS AND blood work. Im FARMED OUT by the VA TO my specialist{ for A year at a time } BUT THEY DONT READ THE BILLS. Its to much to hope for that they got everything right.OH but I have them pay travel PAY to all my private appointments EVEN THE EMERGENCY. We will see whats next===== OUR ICE JUST WENT OUT ON OUR LAKE THE DAY BEFORE YESTERDAY---- STEVE & PAT

  12. Just got my statement the other day-$300,000 exemption on house so the total is $66.00 solid waste fee- Every thing else is 0s.I can handle that. But I doubt if ill get a property tax rebate thou. Nice to have some relief here in MINNESOTA. STEVE & PAT
  13. I know that in the 80s when we were getting a employability assesment for social security the person I hired said that the last study he had done just won unemployability for a 76 year old military retired lady that applied for service connection . The VA tried to say no to the unemployability but the rating officer was shot out of the water .Said there was no age limit these days. So I used him and the the third time the SS whent right thru. The cost then was $600. dollars. The best part was the hearing officer chewed the SS people ,and there lawyer out for delaying my SS disability for no reason. He told me then & there i was getting SSDI. (They didnt share the doctors statement.)THEY GOT CAUGHT. STEVE & PAT
  14. HEY instead of turning the list over to MANAGMENT====== just have them GET NEW MANAGMENT--- You know i just got the denial letter today SIGNED by the same PERSON that signed them in --1971 and ever since. Steve in minnesota
  15. Ours are high here in minnesota----- I THOUGHT=== I got this years estimate but every line had -00000000000s I dont even know if they raised my property value. But the school taxes are high and will be on the statement. The house and garage & SHED ( toung &groved pine interior-loft with queen matress downstairs sofa table& chairs refrig microwave oh and air conditioning)But its only 10 x 14 feet . Company loves it. Taxes were $1200.00 last year so I got the $300000.00 value deferment this year. WE have about a little over a acre and were on a 2 1/2 mile long lake (TRILLIUM) at about 55 feet deep. its nice BUT YOU HAVE TO PUT UP WITH THE WINTER---IT WAS -35 below zero last week . But its been in the 40s &50s lately (THANKFULLY) I GUESS I WONT COMPLAIN ABOUT MY TAXES+ a few more weeks the ice will be off the lake and itll be bass- crappi northern pike fishing time. The ICE IS STILL OVER 2 FEET THICK. STEVE & PAT
  16. On my hypertension medications and others that dont work or the VA tries cheaper drugs=== I get it NOTED IN MY RECORD I AM ALLERGIC TO THESE DRUGS. Then the PHARMIST- has to supply the correct drugs. Minneapolis tried the same on my service connected hypertension. BP went way up. So now my VA file shows that I am allergic to that BP med.-an intolarance to STATIN DRUGS( chlorestoral drugs)= and allergic to regular INSULIN (use synthetic insulin.) USUALLY THE PHARMISISTS CHANGE THE PILLS__ YOUR CARE PROVIDER HAS TO BASICLLY WRITE A PRESCRIPTION ON WHY YOU NEED THE OLD MED. (non working raises your BP.)Then the pharmacy cant subsitute. I just went thru that with them subing eye drops instead of eye SALVE to kill staff- AND for a Coszar BP pill 100 I WAS TO SPLIT IN HALF {teardrop shape)so finally got 50s. IF THE MEDS DONT WORK YOU GO ABOVE THAT PERSON. steve & pat
  17. My husband is on here almost every day. He is 100% (since Dec. 1994), but called 80% and unemployable to make him receive 100%. He is called at the VA,: "one of them", which to me is intimidating and degrading and disrespectful to a person who served for his country and has not be able to do the "things" other his age can do since he got out of the service--- HEALTH INSURANCE HAS BEEN ONE WE could never get for him or our children since he got out in 1970. I WANT HIM TO BE LISTED 100% WITHOUT BEING A "ONE OF THEM". We have claimed Agent Orange for years. Even though he has most of the things on the list, we get he was in Guam NOT VIETNAM! I have his claim open again, and got the HAAS thing. So I am looking deeper into things. I wish I had found your Jet Fuel Exposure posts in 2006. Steve was considered the best Jet Mechanic in his squadron at the time. He was always a natural mechanic, and did major overhauls in Guam for 18 months he was there. Then he was stationed at Miramar NAS. I FOUND UNDER U.S. RADIATION SITES IN CALIFORNIA THAT #78 IS MIRAMAR NAS. IT SAYS CONTAMINATED SITE, NPL SITE, RAD WASTE BURIAL. Upon searching Agent Orange NAS Miramar came up! Steve and alot of other sailors ate their lunches at these contaminated sites everyday, and hiked many of these areas with everyone els, and they played with the desert fuana and the bugs (he was 19-20 years old, and still a kid at heart). In Guam he was at the NAS in Agana, Guam (May 1967 to Nov. 1968). (The first month he was at Fleet weather central-- where he was sprayed a couple of times with DDT will they waited for the outdoor movie to start at the football field on base.) His job at the NAS, he was attached to the power plants shop as a jet engine ground crew member whose positions were to wash, maintain (overhaul engines), and inspect aircrafts. The work area-- a test cell made off the edge of the Tarmac in a small enclave surrounded on tree sides by jungle which was sprayed back with defoliants (A.O.-- DDT????). It was used to protect them against malaria, and other insect borne diseases. This spot in the jungle was just North of their power plants building about 2-3 city blocks. There were 3 of them who would set up and run the jet engines in their test cell on their "test rail". Their daily job was to rebuild the engines. They actually climbed right into these 26 foot long engines. Most of the damage was from birds flying right into them, shrapnel damage, or bullet hole damage from being shot in Vietnam. These engines were made so they could be fitted with wing tanks. I believe they were the planes that were spraying the Agent Orange on Vietnam. They would test run these J57 turbo jet F8 engines after they were finished doing the necessary repairs they needed. This included all the electrical and fuel lines, hydraulic pumps, did spray test patterns and cleaning on the 6 canister nozzle set up (in main part of engine), replacing all the carbon seals, stators, and vanes from the intake all the way back to the combustion chambers, prior to testing them by firing the after burner, and then canning them, and shipping them back to Vietnam. Steve said there was usually about a gallon of JP5 fuel in the fuel pumps that would get all over him when he removed it while repairing the engine, and it would peel the skin off his hands like a bad sun burn. ((Steve said the last month he was in Guam one of the 26 foot engines broke it's holdbacks when it went into after burner. It was shut down immediately, but it still "FLEW"--- it landed 12 feet in front of where the intake originally was. DOD investigated this case as a possible terrorist attempt, and sealed up the area for days. So THAT has to be soemwhere on the DOD records. It was later classified as an accident.)) Just recently we realized the carbon ash residue from the burnt fuel in the combustion chamber, back through the complete afterburner system (tail pipe)---- actually is carbon vanadium ash residue which when I see the health effects MAY BE THIS WITH THE DIOXINS OF GUAM AND MIRAMAR. Since Steve was in Guam in 1967 to present ((((he just seen the local eye doctor today BECAUSE THE VA SENT HIM MORE EYE DROPS, INSTEAD OF THE SALVE THEY GAVE HIM FOR ABOUT 2 YEARS-- TOO EXPENSIVE NOW-- GIVE HIM DROPS INSTEAD!! The doctor gave him a prescription for salve today and it costed us $4.00))))) he has had sores, rashes, lesions, anal fistula, lumps, all over his face, neck, in ears and nose, arm pits, crotch (((VA says it was just teenage stuff, and claim they don't see it, so they have taken skin problems off as service connected). Our 6 year old granddaughter was born with the start of an eye sore. At 4 days old she was put on the same salve as grandpa. The doctor said he had never seen anything like it. We are going to claim aspestos exposure too this time for the Med Cruise on the FDR in 1970 (the FDR had been in Vietnam just before the cruise Steve was on.), vanadium from jet engines, contamination on Guam. If anyone can think of something more we can add, we would appreciate any good ideas, and or information, that we should put in. The 2 other sailors he worked with he believes were: ADJ 2nd class (?) Burt, and Airman Kent Davis. Thanks, Steve and Pat
  18. HERES MORE OF THE CALIFORNIA NEWS STORY OF DOD POLUTION [[[[[[[[[ ITS NOT GOING AWAY- ITS STILL MOVING----- STEVE & PAT Feb-10-2009 09:26 <h1 class="title">TCE-Related Toxic Waste in Irvine Much Worse Than Previously Revealed</h1> Tim King Salem-News.com A plume of the highly toxic chemical trichloroethylene that originated from the El Toro Marine base, has migrated beneath most of Irvine. Image courtesy: eltoronow.com (IRVINE, Calif.) - Salem-News.com began a series of reports in May of 2008 on the toxic waste contamination of the now-closed El Toro Marine Corps Air Station in Irvine, California. Our reports increased in frequency, and then we added more writers who specifically addressed the contamination stemming from the Marine base. The Orange County press is covering the El Toro subject closely now, and an environmental coalition has announced plans to build a Racketeering case against the developer Lennar over their plans to build a park and series of homes on the hazardous land. (see: Groups Prepare Civil RICO Case Against Lennar Corporation for Civil Racketeering) A 2001 document Salem-News.com received this week that originated from Don Zweifel, MCAS, El Toro charter RAB member & pro bono consultant to El Toro Local Redevelopment, indicates that the underground "plume" of TCE moving underground westward away from the old base, had traveled six miles, rather than three as previously indicated. Research underway today indicates that the plume has traveled a significant distance since the six mile mark was noted in 2001. As it turns out, numerous Marines are ill and many have died of diseases that are directly related to the main contaminant on the base; a chemical called trichloroethelyne, generally referred to as TCE. This chlorine-based chemical degreaser was used to strip grease and oil from Marine Corps jet fighters for decades at El Toro and then often dumped through grates into the groundwater. Dr. Phillip Leveque, a retired Oregon Physician who still testifies in court as a Forensic Toxicologist, says TCE is an extremely dangerous chemical that people should never be in close contact with. He testified in 1974 in what he believes, was the first TCE-related death that involved the manufacturer, Dow Chemical, being found guilty and responsible for the man's death. A supermarket janitor, the man who died from TCE contact in 1974 lost his life only 8 weeks after he began using trichloroethylene without a face mask. "His liver failed," Leveque said. "His family took the company to task because there was no warning for this man or others who used it." A former El Toro Marine named Matthew Bonura wrote to Salem-News.com February 2nd, explaining that he was stationed at El Toro between 1996 and 1998. "While I was stationed at El Toro I came down with skin cancer I was only 21 or 22. The base Doctor was shocked to see someone at my age with skin cancer. Now later on in life I'm in my 30's and have a lot of skin problems and my body no longer makes testosterone on its own. I have been to many doctors and they do not know why this is, could TCE be the cause?" Bonura asked. Matthew Bonura's letter is only the most recent from a former or current Marine, that we have received since this series of reports began last year. Another former El Toro Marine named Glenn Hawk also wrote to us in January, explaining that he was stationed at El Toro from 1971 thru 1973 as a Marine Ground support Hydraulic specialist with VMGR-352. "I worked with a private contractor who used TCE to clean the exhaust trail on the C-130 turbo prop aircraft. I saw crash crew use spent TCE in 55 gallon drums, dump it into pits and catch it on fire so they could practice putting the fire out. I saw TCE being dumped down the drains." This story echoes of many others that have been sent to Salem-News in recent months. Dan Perry, a Marine stationed at El Toro on and off from 1979 – 1983, told Salem-News.com that his last duty was with MWSG-37. That is the exact same squadron I served in, during part of the same period. Perry said, "I was sick for several weeks back then. sick bay had no idea what was wrong with me. still to this day have stomach problems. now that I think about it I spent almost 3 years total in the 37 area having lunch at the little chow hall almost everyday. (see my article showing the exact area where Dan Perry and I were both stationed during those years: Irvine, California Threatened by Contaminated Water From El Toro Marine Base) Thomas Clark, who was a Marine at El Toro between 1981 and 1984, says TCE isn't the only problem on the old base that was built during WWII. "There are even more toxic chemicals on El Toro than TCE. I worked at the aviation ordnance compound that supported the flight line. At that compound back in the 1980's we still made Napalm using composition b and composition c mixture." He says he personally witnessed major spills of the toxin within the work area of the ordnance compound. "If the wind was blowing in the right direction you could smell the chemicals in the area. The spills were cleaned up by the munitions crew by washing off the pad with the use of kitty litter (quick sorbwater) and water but you could still smell chemicals in the area. The pad was adjacent to the runway 2R and I believe it was at the end near a field." Yet another Marine, Chris Kelly, who served with VMA-214 Black Sheep Squadron from 1985 to 1988, said he spent many hours with my arms, elbow deep in that solvent. "I remember being amazed at how quickly the solvent would evaporate off my arms when I brought them out of the soaking drum. When our squadron went on 'WESPAC' deployment we hid several 5 gallon cans of 'TCE' on a 'EMBARK' pallet and had it flown ahead of us to Iwakuni, Japan. While in Japan I was told to 'soak' some 20mm gun barrels. One of the barrels' slipped from my hands and landed in the tray, the 'TCE' splashed into my face and eyes." Kelly says his fellow Marines rushed him to an 'eye wash' station and began flushing his eyes. As this took place, the Navy flight surgeon was notified. "I know he continued flushing my eyes for an hour and during that hour he kept asking me where the 'TCE' came from and who told me to use it. I refused to give up my Gunnery Sgt. That was the last I ever saw of the solvent." This last quote indicates that the military did have a thought on the toxicity of TCE more than twenty years ago, and yet it still hesitates to contact El Toro Marines and let them know what they may be faced with. We hope in the context of these reports, to hear from one of the Navy doctors who served around TCE cases.
  19. GREETINGS I was researching our contamination on GUAM and MIRMAR BASE IN CALIF and ran across a news outlet on superfund sites the MILITARY is NOT advertising IN CALIF & ETC. Now NAS AGANA IN GUAM and ANDERSON AIR BASE have the TCE _ PCE chemicals at HIGH RATES in there aquafirs . MAYBE IT MAY HELP US. I DONT KNOW IF THIS WILL WORK YOU MAY HAVE TO PASTE THE ADDRESS in your brouser . LATER STEVE & PAT http://www.salem-news.com/articles/december292008/el_toro_ro_12-29-08.php ://http://www.salem-news.com/articles/...o_12-29-08.php
  20. I kind of stole or checked out a 1971 X-RAY ( it was still there ) of my lower back at the VA in about 1990 .I had it read by my local doctor and a radioligist(sp). I had the claim open and a personal hearing coming up.Long story short it was connected at 40 % because the hospital didnt care for the STRAIN as they called it and it deteroiated over the years due to there inattentiveness. Then later a mri verified the damage. BUT IF YOU CAN GET THE ORIGINAL X-RAYS-JUST CHECK THEM OUT AND KINDA WALK OUT WITH THEM. Have your doctor look at them. Usually the VA destroys them after a few years. The va got mine back -but I had the local hospital make me full copys-when they found out they were for VA claim THEY DID IT FOR FREE=== STEVE & PAT
  21. HI- Im still working on my AO claim -even though I wont gain anymore cause Im at 100% now. Because I was in GUAM--- The VA says our C&P team at the minneapolis VA says THERE WAS NO AO IN GUAM ACCORDING TO DOD++++++++++++++ .They say i worked in the shop so no contamination.BUT we were also a three man crew who ran EVERY ENGINE BEFORE IT WAS CANNED AND SHIPPED BACK TO VIETNAM. OF COURSE THIS WAS DONE IN A LITTLE CONCRETE PAD IN THE EDGE OF THE SPRAYED JUNGLE OFF OF THE EDGE OF THE TARMACK. So Im trying to connect my DMII-VASCULAR HEART DISEASE=CLORACNE=THRYOID-PERIFIERIAL ARTERY DISEASE-AND THE LIST GOEWS ON 'AM already connected for HYPERTENSION WITH KIDNEY DISEASE-LEFT FOOT DROP-HEARING LOSS-LOW BACK INJURY= RHINITUS-SINUSITUS=and TDIU P&T. Will be NOD-ing shortly Am contacting others from my power plants shop at NASAGANA GUAM to get there medical info. The island has been contaminated since WWII with herbisides & DDT since then. I remember them spraying DDT for the MOSQUITOS before the outdoor movie at the USNAS AGANA air station & NAVAL STATION . ALL this poision. STLL WAITING ON HAAS STAY IN GUAM STFVE & PAT
  22. nature- the VA did the same to me last year----THEY LOST THERE COPY- I mailed one copy -1 copy thru my poa -1 copy thru my service officer-(hand carried to the VA +hand delivered by POA= signed recifpt & dated============ YOU CAN ALSO FILE THIS OVER THE PHONE ALSO AS I DID TOO, by calling the va at Your federal building---- NOTE -- THEY DONT EVEN SEND A SELF ADDRESSED ENVELOPE ANY MORE----- STEVE & PAT
