betrayed
Betrayed, what professional organization developed these forms? Is it recognized as a "legal tool" in making mental evaluations (and for VA purposes)? I haven't seen it before. Also, a tool like that needs to have a scoring system that relates to the severity of disability, e.g., severe, moderate, etc. Where is that? Thanks. PS. The last page on the right didn't seem to copy/paste properly. Can you edit your post and fix it, please ? I have no idea, my lawyer specializes in SSDI and workers comp. The last page looks fine if you click on it and make it full size, there is nothing there but lines and the two words date and signature.
