The VA typically wants a form for each medical provider, with you adding the providers name, address, etc. Seems they are too lazy to fill out the forms when you have already provided them with the medical provider info. Naturally, they have an internal form that they fill out using the form you filled out, and send it along with the form you filled out and signed.
