i get so whimsical when i read that bert. va west haven did admit to cloning notes (copy and pasting) these were for physical exams. the falsification of note (preop exam and informed consent discusssion is actually allowed by an addendum of sorts to vha 1907.1. a new note is allowed to be written in the medical record to extend h @ p exams. so despite not having an appointment and holding an invoice that im 20 miles away for the exam date and time...these a.h.s. get away with the falsifications.
