My Question is this a standard input or are they saying it appears I have these symtoms and need to be treated

Problem 1: PTSD as manifest by....

1. recurrent and intrusive distressing recollections of the tramantic event

2. recurrent distressing dreams of tramatic event

3. intense psychological distress at exposure to cues

4. avoids thougts of feeling or conversation associated with the trauma

5. feeling detachment or estrangement from others

6. sense of foreshortened future

7. difficulty fall a sleep and staying a sleep

8. irritability or outburst of anger

9. dfiificulty concentrating

10. Hypervigilence

11exaggerated startle response

He has a short and long term goals established

Is the above information a positive input for my rater to review my PTSD claim

MSE: Wearing T Shirt that rides up on his sotmach, Mood mixed some depression and mild hypomania. Affect anxious and appropriate, NO SI presently. NoHI prensently. No A/V halluc+ ongoing near paranoia at times re: his vulnerability ( feels a need to be constantly moving) Judgement appears good. Insight appears to be fear

What your feeling, advice on the above, becritical, because of Meds, I won't take personally, lol

Are these adequate progress notes

Stressor has been verified and C&P was done

This is progress notes by Social Worker at a VA Clinic that I assume the rater will see

Are they adequate, and will the rater use this type of information to determine my rating

I hope so- I am glad the stressor is verified- as to the social worker notes-

they might not be in the C file-but most of the time they are-

I wouldnt worry Sgmdae as long as the C & P exam is there and the diagnosis and treatment records for the PTSD.

It never hurts to send them copies of stuff that they might already have-you can use a 21-4138 form at the VA web site as a cover letter telling them to use this record as part of their rating process for your claim.

I wonder how the Social worker knew the stressor was verified- I imagine they had something from JSRRC in the file that the social worker saw?

This sounds good-hopefully it wont take much longer before you get a decision. The Waiting is always just awful.

Yes, the stressor verification is the thing. PTSD DX and stressor verification. Now the only other thing would be the IMO's just to make sure you get the maxium rating.

Waiting is hell if you are out of work, but if you are then you file for SSD. The social worker says you have short and long term goals estabished. What the heck does that mean? She is describing a vet with very severe symptoms, but the vet has estabished short and long term goals....huh. Treatment goals????

The social worker gave me a list of Long Term and Short Renge goals

examples

Long Range

Reduce the freguency of intrusive recollections of the trama

Increase interest/pleasure taken in activities

there are 10 of them

Short Range

Pt will attend one education session on relaxation techniques by 11/30/09

Pt will verbalize understanding prevention measures to avoid relapse by 10/30/09

Therapueutic Interventions

Primary Theapist...Social worker...minimum of one visit per month

Psych Medication Mamagement psychiatrist monthly

He was a Combat Vet, His work and relationships have been unstable for years

I think these are good comments for a Rating, What do you think

My C&P examiner did such a poor job, she never recorded the facts, she seem to want to write what she wanted to hear. She didn't capture my terrible work history, dismissal, over 30 jobs, counseling letters etc

Well I guess, this information for the rate increase request of NOD, just hate to go through that

I am jumping ahead, I am trying to relax and wait for the official paperwork and go from there
