Speaking only for me, it's hard not to take certain terms personally when I see them in a diagnosis. In this context, stable or unstable refers to your PTSD symptoms and their severity. If Jane has PTSD but her symptoms are well-managed and her scores on assessments are consistent, Jane's PTSD symptoms are stable. If John's symptoms are not well-managed, his assessment scores are all over the map, and his day-to-day functioning varies widely, John's PTSD symptoms are unstable. It lets your clinicians (and rating officials, should you file for a rating or increase) know that these conditions are sufficiently severe that their symptoms are not controlled from day to day.
