SUBJ: VA Form 21-0845, Authorization to Disclose Personal Information to a Third Party

Amended Guidelines

This fast letter rescinds FL 09-51 Revised, dated December 22, 2009.

We changed the instructions for VA Form 21-0845 to make clear that forms for incompetent claimants and beneficiaries must be signed by the fiduciary (see page 3 – Incompetent Beneficiaries).

We also deleted the temporary flash, CO Special Status 1, instruction and substituted the new flash, Third Party Release (see page 2 – Processing Procedures).

Purpose

This letter discusses the use of VA Form 21-0845, Authorization to Disclose Personal Information to a Third Party.
