Disability Benefits Questionnaires (DBQs) are downloadable forms for Veterans to use in the disability evaluation process. DBQs can help speed the processing of compensation and pension claims.

DBQs allow Veterans and Servicemembers to have more control over their disability claims process by giving them the option of completing an examination with their own healthcare provider instead of at a Department of Veterans Affairs (VA) facility.

DBQs enable private healthcare providers to capture important information needed by VA to accurately evaluate and promptly decide Veterans' claims for benefits.

More than 70 DBQs are available that use check boxes and standardized language to streamline the process. DBQs average about five pages in length. Veterans are responsible for any fees their private provider may charge for completing a DBQ.

How to Find and Submit a DBQ

The DBQ process involves four steps:

  1. Access the form online and download it;
  2. Have your healthcare provider complete the form;
  3. Save a copy for your records; and
  4. Submit the form to VA.

I cannot get to the C&P Exam Worksheets when I click on the linjs provided by several links. I keep ending up with "COMPENSATION 101". Where do I go from there to access the worksheets? Thanks

The problem I'm having is finding a Dr willing to fill out DBQ form.  I've seen two private orthopedic docs and they both look at forms like it was the plague.  Both stated they don't have time to fill out 10+ pages.  That's just for one condition, I had three forms needed to be fill out.  I told my VSO this and asked her to just put me in for C&P exam.  She  then told me I need to find a dr willing to fill out form and more evidence my conditions  have worsened. Very frustrated  and feeling lost

you might think about getting a private specialist board certified IMO, It maybe cheaper than you would think, it don't take a DR that long to write out an impression letter or give his/her opine

if he/she exams you and you explain to him what you need & him/her read your medical reports or at least scope through them to the parts that you need pertaining to your disability's, they can put together a letter of what you need to win your claim.

 just call around and find a Veteran Friendly Dr and let him/her know what you need..it don't have to be a DBQ  just let the Dr know what information you need for your claim  be open and honest is the best way.

VA Doc's are afraid to get involved with compensation claims  they think they maybe fired for helping a vet with his claim...(just my opinion)...on the other hand some VA Dr's Are glad to help.

It don't hurt or jeopardize a VA Dr one bit to fill out a DBQ  it could be they are just flat lazy.
