Reminder To Submit 21-4142 To Get Claim Processed

Called 1800 & checked on status of my claim, was told the 21-4142 is needed to complete the claim. Rep said the form was sent here twice and that was the holdup. My goof. Thank goodness, just found important data to submit and will handcarry now - Prayers please! cg

Makes me wonder if we vets need a simple paperwork checklist like this? .......
