It is probably better to be more specific about why the decision regarding your exact condition is wrong. You can keep most of the body of that sample NOD, but add specifics about your condition as to why it should have been rated at a higher level. I believe Berta actually won an increase based just on the NOD. Keep it to one page since they don't read beyond that. I would also ask for a DRO Hearing.

Use VA form 21-4138

How you write it depends on several factors.

If you want them to reconsider, add that to the form. If not state it is a notice of disagreement.

Explain the date of the denial and the issue or issues involved.

Explain why you disagree. Ask for what you want to reslove the issue and attach any regs that back up your view.

Then sign it and send it certified recipt at the post office.

You will receive an SSOC ithat gives you the option for a DRO hearing / review or the BVA.
