I copied this from Dr Bash's column in the IMO section Forem and would like someone to decipher what this means exactly. Thanks

Thank you carlie! Your so great!!!..Correct me on this if I'm wrong...Subjective=Dr's dx and reason,...Objective=Dr's disagreement on C&P Exam,...Assessment=Dr's review of conditions...Plan=Dr's plans of approaching conditions.

Am I correct with this or????

SOAP is a lot better than a doctor friend used:

I: Impression

T: Treatment

I couldn't believe it when he told me until he shown me a medical record.
