Dbqs On Qtc Medical Questionaire

Question

I have another post about my C&P for my Back, so I will not repeat the same stuff. I have noticed that off the side of my QTC forms in small letters DBQ and it will have lower legs, knees, etc. next to it. I found a web-site, and opened up the form for each DBQ listed, and it looks like it is something for a Doctor to complete. Good Lord, and they gave me less than a weekexcl.png

Thanks for the link, I also have a link that explains the DBQs, but my issue is the very short notice. I am sqribbling all over the form because of my PD. They have Back questions, and next to it DBQ lower legs, etc. The space vs the info required just do not correspond. But, I will do the best I can, and I will be at that exam tomorrow come hell or high water.

Thanks. Very nervous because I was not able to complete the stupid QTC form were it is readable. But, I'm taking my records and will tell them if they can fill it out within the space given, good for them. I can not. I will do my best.
