Insurance Questionnare

Since I have no rating and have never been seen at a VAMC for anything other then a C&P exam is it absolutely necessary that I fill this thing out?

I guess you mean the insurance questionaire at the VAMC, where they try to bill your insurance company. Its up to you, but I probably would not fill it out. It may, however, help other Vets if your bill is partly paid by your insurance company, however.
