Greetings and Salutations, all!
Long-time lurker, first time poster here. Just completed three of my C&P exams (PTSD, General Medical, Hearing/Tinnitus) and still waiting for the last one to be scheduled (filed for IBS/GWI claim, but thinking I might be closer to chronic diarrhea/sphincter impairment. We'll see what the GI doc says).
Would appreciate any opinions/WAGs on the three main claims. I'll make this as readable as possible.
Initial Post Traumatic Stress Disorder (PTSD) Disability Benefits Questionnaire
SECTION I:
1. Diagnostic Summary
Does the Veteran have a diagnosis of PTSD that conforms to DMS-5 criteria based on today's evaluation? [X] Yes [ ] No
2. Current Diagnoses
Post-traumatic stress disorder, chronic, severe, from daily fear of hostile military and terrorist activities
3. Differentiation of symptoms
Does the Veteran have more than one mental disorder diagnosed? [ ] Yes [X] No
4. Occupational and social impairment
Which of the following best summarizes the Veteran's level of occupational and social impairment with regards to all mental diagnoses? (Check only one)
[X] Occupational and social impairment with reduced reliability and productivity
SECTION II:
1. Evidence review
Was the Veteran's VA e-folder (VBMS or Virtual VA) reviewed? [X] Yes [ ] No
Was the Veteran's VA claims file reviewed? [X] Yes [ ] No
2. History
a. Relevant Social/Marital/Family history (pre-military, military, and post-military):
His biological parents divorced when he was 3 years old. He was then raised by his mother. He has a sister. He describes an unremarkable pre-military history. His first marriage lasted from 1999 to 2002; He has an 11 year old son from this marriage. He maintains contact with his son. He has been married for the second time for 9 years. He has a 6 month old son. He has detached and inhibited relationships with his wife and son. His wife is also in service. Their postings, frequent change in work stations further affect their marriage life. He is socially withdrawn.
