Berta
"SACRAMENTO - The Veterans Administration confirmed that a "do not resuscitate" armband was mistakenly placed on a patient who died following surgery Friday at the Sacramento VA Medical Center, but insisted the error was not fatal. Family members identified the patient as Roland Mayo, 65, a Vietnam veteran and former Riverside County deputy sheriff and marshal." Hope they got an autopsy because no matter what the VA spin is, they surely have a basis for a FTCA case. I thought DNRs were supposed to be in the med recs, if the veteran had one. This vet didnt have one there.http://www.news10.net/story/news/local/sacramento/2014/10/20/veteran-dies-after-armband-error/17642173/
georgiapapa
Sounds like another VA OIG investigation is warranted. I would like to know if he went into cardiac arrest and if they attempted to resuscitate him. What did the VA claim as the cause of death? If the VA can not produce a DNR signed by the deceased vet, they are up the creek if it turns out the vet died from cardiac arrest as a result of the VA failing to attempt resuscitation. The family should definitely get an independent autopsy if it is not too late.
