just wanted to add this . if you are getting treatment through Choice

what or who ever is treating you has to ask for additional treatment

not VA provider the person who is doing the treatment has 2 papers to send to

Choice Your medical records and request for additional treatments. Not your

PC.

You have to take care of this and not VA once you are approve for outside care leg

work is on you. Before your treatments end always ask the doctor for more if you need

it. I put in 4 weeks before my time expire and my treatments continuing with no

problems.
