Go on your MHV account, hit the BLUE Button. Request a customized report, indicating the time periods of your Tests and your PCP responses.

Select all the Tests Results and Clinician Treatment Notes, that should do it. Download & Open.

If you need a specific interpretation for something you don't understand, send your PCP or Specialist a Secure Message regarding same. I usually get an answer back by the end of the day or the following morning.

If you can't find what your looking for, a trip to your VMC Med Recd Department will be required. In 10 min you can have the exact pages regarding test and Clinician notes. While your there, ask to have your Complete VMC Med Recd provided on CD. Might be able to get it while you wait, if there not busy. I think I received mine, about a week after I signed the release requesting it.
