Medical Records- how to get-

Rate this question

Berta

Question

I get frustrated everytime I see a veteran using FOIA to get their medical records- they might come a lot faster if you use this form

https://www.va.gov/vaforms/medical/pdf/vha-10-5345a-mhv-fill.pdf

You can fill it out on line, and send to your VAMC ,or give it to them in person  or copy and give to your attorney ,if you have one.

You dont need FOIA at all- these are your records.
