What is a Temporary Rating vs P&T

What does the VA Mean by this and what is the Intention?

When a Veteran is rated either  S.C.100% P&T scheduler or even 90% and given the  TDIU  with P&T with his disability not expected to improve in his life time and no future exams scheduled.

Okay as the years go by say about 13/14 years veteran is concurrent on his S.C. disability's  and this Veteran comes up with another condition caused my his military service and he files claim  for this & Awarded a 70% rating (keep in mind he has a 100% P&T Rating) Over 10 years.

Ok the VA looks at the SMC Rating tables and gives this 100% P&T Veteran the SMC S H.B.   via Commandant Rating (maybe this falls under the duty to assist)  or mandatory, never the less  my question is

Is this new rating with the SMC S H.B. a Temporary Rating? do they come after this Veteran around 4 years later to reevaluate  this Veteran? is this why its only a Temporary Rating?

IF YES?  Then What happen to the ''not expected to improve in his life time with no future exams scheduled?''

or do they need to quote this  new rating as  P&T And this disability is not expected to improve in his/her life time and no future exams scheduled  on every rating a veteran gets? After the 100% P&T Rating?

if this is true  then all of our ratings is only Temporary. (excluding death of course)
