Form for Mental Health IMO

Is there a specific VA Form that is used for a mental health disability (it's not for PTSD)? My psychiatrist will provide the iMO but wanted to know if there is a specific form that is used. I searched but could not find one (I probably just over looked it).

I believe its the 21-0960p-3  and not the 21-0960p-2  the 21-0960P'3  is for other MH dis-orders...other than PTSD and eating disorders.

I am looking at the form 21-0960p-3 now. Will my doctor know the ICD codes and Axis Category in Section II? Sorry to ask such things, it just when I was talking to my psychiatrist she had no idea what I was talking about.

you need to the 21-0960-P-3 ....> Not the 21-0960P-2

 

Paul your Dr may or may not know the ID CODES  some do some will look them up  but if you know them   uses them write them down on the form some place  just mention in a good  way  just say ''here is the Codes for*****  In case you need them? I looked them up    or something close to this.

All the information we can feed them is ALL GOOD.

It is not for PTSD so 21-0960-P-3 would be the correct form and those questions are on that form.
