You may need to go to an IME/IMO doctor.  Most doctors will not do one of these if you visit and ask.  I found this out myself and have heard it from other vets.  I found a doctor in the next town over who did mine but it was a bit steep, $1500.  Try:

Make sure that the will write up the report correctly.  That your disability is as likely as not connected to your service and that they have read all of your records.  That's where it gets steep, a good doctor will read all of your records.
