Yes indeed it is very very powerful especially if you bring it to the Dr. Office who has pledged to uphold his/her commitment of oath should document this in my healthy vet chart!

It is Supernecessary and indeed helped me shed light and approve my case.

dodger There is no special form at least, that I could find. Simply start tracking the date and time of the headache, how long it lasts, some description , if you took any meds for it and, if you had to prostrate or lay down in a dark room. If so, for how long. Record every event, including several times a day if it happens. Give a copy to your doc, and include it in your lay statement (statement in support of a claim.) It is critical because lay evidence is the only way it can be documented.

You can find many different kinds of diaries on the net.  Your diary carries weight if it is added to your medical records.  It is your self reporting of your migraine condition and is considered important evidence unless you impeach yourself by being caught in dishonest reporting.  I say this because I had a doctor try to impeach my evidence but he was shot down, they may try.
