SMC L? should I be rated higher to SMC M

Question

I received my award letter today and was granted increase to 100 ptsd/mdd/somatic syndrome disorder. 
previously I was awarded smc L in April for being so helpless,... but didn't have a single 100 rating in April... the VARO made an error...so now  I think the VARO corrected the mistake in my April award and corrected it in my July award because I have a single 100 rating for ptsd...
Question:
My previous award or current award didn't include a Rating Decision Code Sheet, so do I call 800# to request this document?
I don't know what disabilities the VARO is including in my helpless SMC L award in my A&A form. 

Question:
If one or more of my disabilities in listed my original A&A form had ratings 60% or lower and in the future they became worse like "loss of use" and I requested a rating increase in the future... would that rating be considered  1/2 step or whole step or additional SMC L or not because it was included in the original A&A form...confused

current disabilities: 100 ptsd, 60 ivds, 50 bilateral feet, 20 hip, 20 hip, 10 hip, 10 hip, 10 tinnitus, 10 tendinitis,
 10% left footdrop, 10% right footdrop, neurogenic bowel control 0% and neurogenic bladder 0%.

Question:
If any disability worsen in the future to "loss of use"...would any of disabilities be rated as a second SMC L?

Great questions!  There are levels of "aid and attendance" (SMC L, but A and A is "not the only way" to get to SMC L) and those are based upon the "level of care".  

Yes, disabilities can be combined to get a higher SMC rating. Without knowing your "level" of A and A, and/or your levels of "loss of use", Im unable to give you more than "general" information.  

No nurse needed:  SMC L

Nurse needed to change dressings, bandages, etc = higher levels of Aid and Attendance.

broncovet...when I receive the rating decision code sheet I'll have more questions about my SMC L rating...right now I dont know what service connected disabilities the varo used to determine my smc L A&A helpless...I thought it would be SMC M, but I think it requires an additional 100 or two other rated at 50 not included in the A&A...confused
