You can ,under the Remarks part , refer them to additional evidence and then enclose it and make sure you name ,address and C file number is on it.

I never use the forms while on line. I always print them off as it is easier to fill them out that way. Sometimes.

I have only used the 21-4138 once in 17 years.

I typed a statement to advance my AO claim and referred them to enclosed evidence and then listed it -all in an Office org doc.,Exhibit A, B, C etc. making sure my Doc. was not too long-then I printed the Office org doc and cut it down to fit into the 4138 area for a statement, taped it to the form , copied it, then scanned it formally into a 21-4138.

Then I printed it all off and mailed it.

Maybe all that above doesn't make sense.....but it worked for me.

TDIU forms are so important that, if the VA allows making an on line application, I still think it is better to fill out any VA form off line, even if you have to do it with your own handwriting instead of typing -

because that way you can refer to the attachments and they would be sent in the mail by you along with the mailed TDIU form.

I don't have much confidence that VA will always be able to associate a mailed in attachment with an on line application.

NARA does that by putting a bar coded print out a claimant needs to sign and mail to them when they request SMRs on line.

I found the NARA online SF 180 application along with the bar coded thing certainly worked well for me.

But I hope others chime in here too- as maybe I misunderstood your question.

As far as the doctor's list , VA will probably focus on the most current ones first..Just do your best to try to give dates, putting the most recent treating doctor's first.

If he gets SSDI and it is solely for SC or what you expect to become SC, tell the VA that too.

And tell them of any side affects from SC eds he gets that could hinder employment.

If VA Voc Rehab ever turned him down for Voc Rehab solely due to his SC, make sure the VA knows that too.
