Others have asked for the form and while I had never saw it, nor is it really needed to get a statement, I decided that I would post it for those that want to use it. NexusForm_VA.pdf Rick, Never saw this form before, but it looks like a great way to convince your PCP or specialist that s/he should write a medical opinion for you. Thanks a bunch!
