Is Caretaker The Same As Aid And Attendance?

I am currently rated at 100% ptsd 0%tbi with a 30% migraine connection. 10% knee 10% spine, 10% foot. Would smc s be an option? -
I am currently rated at 100% P&T back dated to 2010. My wife receives tier 3 caretaker. I am not sure if there is a difference, but if there is what is the next step in applying for aid and attendance? will they back date my pay to 2010? Are they going to try to find me incompetent?
