How do you get on the CAVC docket? What form do I use?

However, when I tried the site was apparently down. Due to this being Saturday nite, my guess is the site is down for maintenance.

If you are filing a Writ of Mandamus, then I suggest you use hadit's sample writ, then cut and past your information. That is what I did.
