What Form Do I Use To Make Corrections To Medical Records?

Does anyone know what form to use and how to make corrections to your VAMC medical records?

I never knew that someone at the VA could help transcribe the chicken scratch handwriting that nobody can read. I'll have to look into that. They are going to be really busy...

I asked my doc to add information that was completely absent, but occurred. The doc said that it was so busy that he cannot remember the details, so he just noted that as an addendum in my records. I guess my chances of a correction are out the window.

Not true, if you contact the patient advocate at the VAMC they can speak to the doctor. The doctor is required to respond to their e-mail. However, if the doctor refuses to correct the entry than the IT security person can override any entry and correct it to your request.

Recently I went to a C&P for a urinary issue that is already service connected but has worsened over the yrs.

Without prior warning, the VARO ordered a prostate examination due to a medical entry in my VAMC health records that is an error. All of my physical & PSA test have been normal so I already know I don't have a prostate problem. My guess is the VA rater is trying to drop my rating for the urinary issue by blaming it on a prostate problem I don't have because of the false entry in the records and I don’t have a claim in for prostate.

Also, I was diagnosed with "short leg syndrome" some 10 or 15 yrs ago by an orthopedics specialist due to muscular atrophy. I wear a lift for it.

Not long after including this in my claim for MS as a secondary issue, I noticed an entry in my health records by my current PCP that I do not have "short leg syndrome. I've never discussed this atrophy with my current Dr & he's never measured me or examined me for it. So that’s another one I want to get cleared up.

I also have entry’s in my records by him as having "white coat hypertension" while he is prescribing medication for it. I had a claim for hypertension also.

Up until recently I really liked this new PCP I’ve had. But if he's only working for the local VA rating dept, I want as little to do with him as possible.

I will fight them to make sure everything that is not correct is changed. Sick and tired of their BS games.

I called around yesterday I was instructed by the VAMC records dept that I need to fill out a "medical records amendment form" and list everything I want to amend in my records. They said to include any supportive information so i'll include my supportive records like 20 yrs of blood pressure readings & diagnoses for hypertension, diagnoses by a specialist who actually measured my right leg & records showing normal exams & blood PSA tests.

It's an absolute shame a vet has to constantly check his records & call them out on their lies & make these corrections, but it looks like I have no choice since it's all about the VARO rating officers controlling the health records.

My advice to all vets is "ALWAYS" get copies of your records, check them over with a microscope & demand the records be corrected. It's either that or spend decades appealing trying to get your ratings restored after they cheat your out of them.
