HadIt.com Elder allan Posted July 30, 2010 Author HadIt.com Elder Share Posted July 30, 2010 I was instructed by the VAMC records dept to make the corrections, include any medical records needed and return to the VAMC medical records Dept. They will then send it to the Dr to make the corrections(if they agree?) and send it back to the records dept for final corrections in the computers. Say for example I have HTN diagnosed for a few decades and my current Dr prescribes medication to keep the water swelling down in my feet and calves. Lets say he also makes remarks in my Med records that I have "white coat hypertention" and I want this removed since I have a firm diagnoses of HTN & prescribed meds for it. So I may want to include a few decades of hypertention diagnoses and elevated vital readings to back up my claim. If their writing down that I don't have HTN at the end of the Dr's visit on retesting for HTN and they are not retesting. Than it's going to be my word against the Dr's and nurses that are just doing what the VARO instructs them to fake. It's their word agisnt mine in that case & I doubt much will be changed with it other than I protested the comment. I also see a private Dr & their vital readings logs will be different. They also don't check beginning and end of Dr's visit, but don't write down that they do either. I tried to print your PDF file Wings, but the page came out small and accross the page instead of down the page. I'm able to print the jpg I posted by just clicking on the thumnail and hit print once it comes up. If anyone is having trouble getting a copy of the form printed out, maybe we can get TBird to set up a link to one that works for all of us.
