ACTION: Notice.

SUMMARY: The Veterans Benefits

Administration (VBA), Department of

Veterans Affairs (VA), is announcing an

opportunity for public comment on the

proposed collection of certain

information by the agency. Under the

Paperwork Reduction Act (PRA) of

1995, Federal agencies are required to

publish notice in the Federal Register

concerning each proposed collection of

information, including each proposed

extension of a currently approved

collection and allow 60 days for public

comment in response to the notice.

This notice solicits comments for information

needed from claimants prior to undergoing a VA medical examination for disability benefits.

DATES: Written comments and

recommendations on the proposed

collection of information should be

received on or before July 11, 2011.

Title: Report of Medical Examination

for Disability Evaluation, VA Form 21– 2545.

OMB Control Number: 2900–0052.

Type of Review: Extension of a currently approved collection.

Abstract:

VA Form 21–2545 is completed by claimants prior to undergoing a VA examination for disability benefits.

The examining physician also completes the form to record the findings of such examination.

An examination is required where the reasonable probability of a valid claim

is indicated in any claims for disability compensation or pension, including

claims for benefits based on the need of a veteran, surviving spouse, or parent

for regular aid and attendance, and for benefits based on a child’s’ incapacity of

self-support. VA uses the data to determine the level of disability.

Affected Public: Individuals or households.

Below is a link for VA forms and I can't find this VA form 21-2545 to even see what it is for the claimant to fill

out PRIOR to undergoing a C&P exam.

I just dug out my file from 1978 and found a 21-2545 -

Report Of Medical Examination For Disability Evaluation.

I sure can't find it on the VA Forms web site and wonder if it has changed

since 1978.

The one I found is 4 pages long and the vet is supposed to fill out and sign page 1,

then pages 2,3 and 4 are for the doc to fill out and sign.

On my I am sure I did not fill it out as the answers are typed in and it does not have my or any

signature on page 1.

I'm wondering if any comments need to be made on this issue because it strikes me a little funny

because the vet would have already filed a 21-526 and this regs states specifically that the information is needed

PRIOR to undergoing a VA Medical Examination for Disability Benefits.

It seems to me that if this form were necessary - with all the claims I have filed and all the

C&P exams I have had - I should certainly be familiar with this form.

Also, seems like others would have made mention of filling it out ? ? ?

Carlie, my claims started in the mid 90's and I never filled out or heard of a 21-2545. My only guess is that because of electronic record keeping that the form is no longer required. My 2 cents.

It's announced in the Fed Register and up for comment

so I would think the form that they say needs to be completed prior to a C&P exam

should be current and available to locate.

I have had numerous C&P exams and I have never seen this form either at anytime during the past 14 years, I havenever filled out a medical history for a VA exam

100% SC P&T PTSD 100% CAD 10% Hypertension and A&A = SMC L, SSD
a disabled American veteran certified lol
