Request A Varo De Novo Review

I just completed my 21-0958, do request the De Novo review within the NOD or does it go on a separate form? Also is it to be sent in with my NOD and evidence? \

I am by no means an expert but when I submitted mine, I filled out the form and put *continue on attachment. Within that attachment (a word document), I clearly spelled out the request for De Novo review. I sent in the form and all my evidence at the same time. Good luck!
