Question

Page 2 of the form asks for the month and year appliance or skin medication was issued(MM/YYYY).

I was first prescribed this skin medication in the US Army in May, 1979, and have been using the medication CONTINUOUSLY since then.

I also was prescribed this medication by a civilian dermatologist shortly after leaving service.

I have a copy of a recent prescription(July,2017) for this medication from my current dermatologist.

 

So which date should be used?

 

The next box asks for the name and location of VA facility that issued appliance or skin medication(if not a VA facility include facility's phone number).

 

Do I give them my doctor's name, address, phone number, or do I give the VA the information on where the product was dispensed?

 

I have been prescribed this medication for many years, but it is also available over the counter.

Is that a problem with the VA ?
