You can upload documents via VONAPP (found through eBenefits).
If you are using a Service Organization, you should contact the NSO office at your VA Regional Office and ask them where to send. Most Service Organizations still want correspondence to go through them.

Could you please post what Series, Rating and Job Title are listed on your Discharge.
Also, I see that you have your Service Treatment Records; but do you have your Official Military Personnel File? Your entrance and exit physicals are normal located in the OMPF.

Do you have any later MH treatment records from the CBOC. The June records don't have an official diagnosis of PTSD.
If you have any of the later treatment records (June - October), could you look for something like this:

Axis I: All psychological diagnostic categories except mental retardation and personality disorder
-
Axis II: Personality disorders and mental retardation
-
Axis III: General medical condition; acute medical conditions and physical disorders
-
Axis IV: Psychosocial and environmental factors contributing to the disorder
-
Axis V: Global Assessment of Functioning or Children’s Global Assessment Scale for children and teens under the age of 18

I just read the treatment record that you posted ( ). I see the date is June of 2014.
I'm curious, when did he start MH treatment at the CBOC? Was the June appointment his first? Has he had any appointments since?

In mid 2014 the national average was 24 days.
My experience has been 30 days or less, for most of the Veterans that I work with.
