A Reconsideration Request with the BVA takes the form of a Motion (this is explained in post here today to Steppenwolf)

and a Recon request on a RO decision just needs the same type of format which can be typed on a 21-4138 form.

The request shold contain new evidence-which could be evidence that VA had but failed to consider at all.

It does not take the place of a formal NOD and if they dont act on the request as the NOD year draws close then a NOD should be filed.
