ACTION: Notice.

SUMMARY: The Veterans Benefits

Administration (VBA), Department of

Veterans Affairs (VA), is announcing an

opportunity for public comment on the

proposed collection of certain

information by the agency. Under the

Paperwork Reduction Act (PRA) of

1995, Federal agencies are required to

publish notice in the Federal Register

concerning each proposed collection of

information, including each proposed

extension of a currently approved

collection and allow 60 days for public

comment in response to the notice.

This notice solicits comments for information

needed from claimants prior to undergoing a VA medical examination for disability benefits.

DATES: Written comments and

recommendations on the proposed

collection of information should be

received on or before July 11, 2011.

Title: Report of Medical Examination

for Disability Evaluation, VA Form 21– 2545.

OMB Control Number: 2900–0052.

Type of Review: Extension of a currently approved collection.

Abstract:

VA Form 21–2545 is completed by claimants prior to undergoing a VA examination for disability benefits.

The examining physician also completes the form to record the findings of such examination.

An examination is required where the reasonable probability of a valid claim

is indicated in any claims for disability compensation or pension, including

claims for benefits based on the need of a veteran, surviving spouse, or parent

for regular aid and attendance, and for benefits based on a child’s’ incapacity of

self-support. VA uses the data to determine the level of disability.

Affected Public: Individuals or households.

Below is a link for VA forms and I can't find this VA form 21-2545 to even see what it is for the claimant to fill

out PRIOR to undergoing a C&P exam.

Carlie I spent my response the the ladys name listed on the announcement site I hope more people complain about them not using it if they did it might cut down on many of the remands and NODs and appeals that are now having to be filed

It's announced in the Fed Register and up for comment

so I would think the form that they say needs to be completed prior to a C&P exam

should be current and available to locate.

Interesting.....

I've never seen the 21-2545 - Report of Medical Examination for Disability Evaluation form before.

I keep copies of everything and didn't find this one in my file. I filed my first claim in 2008 and

have had several C&P's since.

At about this time last year, the C&P exam I went to was based on computer program entries by the examiner. I was not permitted to see the entries.

The display screen was deliberately placed so that the veteran would not be able to read the on screen data.

Here's a link to the form. Whether it is currently used or not, I don't know. I found it via a search engine as well as on a VBN topic (same link address for form).
