Thank you for providng a wealth of information and support.

I finished my military commitment in 1990 and entered the National Guard where I was discharged as medically unfit. Prior to my ETS date from active duty, I completed paperwork stating all the medical concerns/problems I had while enlisted.

Does anybody know what form this is and how I can obtain it?

There was one medical concern I had and it was never considered or addressed. (Ongoing dizziness from the last day of basic training until I was discharged. They said it was a stomach virus.)
