Question

I received the attached letter.  They say they want me to fill out and send them 21-526EZ.  Is this a typo?  I assumed so and filled out a 21-0781a and uploaded that to ebennies.  Do I need to mail or fax it as well?  Should I fill out another 21-526EZ.  My Jaw Condition is arthritis aggravated by clenching and grinding my teeth FYI.

You should send them what they are asking for..... even if you filed secondary a on the proper claim form  which is the21-526ez....  it might got lost?

or they need you to file what you are filing on the  21-526 ez  they probably need the claim on the 21-526ez  to go into the new digitize system to further process your claim.

actually your lucky they didn't deny your claim if you sent the first one in  on the old claim form ?

even if you file a secondary claim to what ever  it needs to be filed on the 21-526ez.
