VA forms 21-4142 and VA 21-4142a

Question

I filed  claim in Jan 2018 through my VSO.  I have been seen at the VA twice since then.  I am getting ready to file another claim but am not using the VSO I had in Jan.  Do I need to file another form VA 21-4142?  And what is the difference between these two forms?  The VA should have all of my records except possibly what was documented just since my decision letter from July.  I just want to make sure I fill out all the proper forms.

It would depend on the type of information you plan on providing to the VA.

VA Form 21-4142 (non-medical info)
Authorization to Disclose Information to the Department of Veterans Affairs(VA) (Fillable)

VA Form 21-4142 (medical info)
General Release for Medical Provider Information to the Department of Veterans Affairs (VA) (Fillable)
