Are Release Of Info Forms Needed?

Question

Ive just put in my application for SSDI online. It says to send in the SS-827 (release of info) forms filled out for all my Dr's and facilities I have been to. My question is, I have copies of all my med records from all but two places. Do I really need to send in these forms for them to get them or can I just send in copies of all my records?

I would think they would accept what you have but always best to check-

does the online site have a "contact us" feature to ask them?

I think your local SSA office (who will probably get your app anyhow) might know-and they should be in the phone book under the gov listings.

Nathan, I tried to do as you are trying and was told that even though I look honest that someone at one time or another forged their medical records so they prefered to get them from the medical providers direct.

As Berta has suggested call the local office and ask. Your office may be different than mine was. Take care. Terry Sturgis
