    OK,  this is an easy fix.  If you can't find copies of the old forms you submitted,  call the 1-800 number and request a blank 21-4192, as well as a blank statement in support of claim. Complete the 21-4192 form yourself,  to the best of your knowledge,  try to be as accurate as possible. On the statement in support of claim form,  explain that you had to complete the form yourself due to prior employers being out of business.  Also explain that you have previously submitted the 21-4192. Be SURE to include this statement,  at the end of the form "I declare under penalty of perjury that I have completed form 21-4192 truthfully and to the best of my knowledge". I did the same thing when my prior employer said they couldn't fill out the form,  and the VA accepted it.  Fax it back into them asap.  Best of luck

    OK guys, something new with this I guess.   I was going through my medical records and it seems my PT ROM measurements last December would have basically qualified me for an increase.
    My cervical Flexion ROM is 10% Extension 5%      R S/B 5%          L S/B 5%  R rotation 20% and L rotation at 20%.
     
    So now that I know this, how do I inform them of it and request that THAT be addressed as well?  Man, smh, I'm a mess!! lol

    Right. Whether it involves a SC or an NSC condition, the VA will pay for any ratable condition that a medication prescribed by VA  had caused. 
    The claim can be filed under 1151-for NSC issues) plenty of info here on that or,if directly due to a SC disability the claim can be filed as secondary and also as 1151.info her on that scenario too in our FTCA and 1151 forums.
    You will need medical proof of the additional disability(ies) and that they are at a ratable level.For 1151.
    If claimed secondary, to an established SC, then even if the VA rates the secondary at "0" SC, it could get worse in time and then be proven to be ratable.
    If a private doctor has said the VA meds caused your problems, and give a full medical rationale ,and they put that  into the form of an IMO/IME here ( topics on IMos available here too,you would have a very valid claim.
