Getting Copy Of C&p

Anyone know how i can get a copy of my C&P report i had done on july 29th 2008 at the va hospital in Irvington NJ????

Go to the Release of Information office at that VAMC and request it. They can print it off while you are waiting. All you have to do is sign a release and get your copy. If it's too far to drive, call the ROI office and request the copy. They will probably have to mail you a release form first, and it may take a few weeks to get it.

Actually, rentaldude, I think (I've actually quit thinking, according to MrsLarryJ) it's to the "Medical Records" window he needs to go. At least, that's what they call themselvs in the Dallas VAMC>
