Which Form? To Start Va Claim?

Question

I have multiple claims that I would like to file for. This will be my first time ever filing a claim. I have in total 4 claims. Which form do I fill out?

Form 21-526EZ (I heard that you can only file up to 2 disabilities on this form) or standard 21-526?

There is no limit for the EZ form, but it is dependent on your providing all the relevant info that it asks for. Either will work, the EZ form just has fewer pages, and I think a few years ago my rep told me that they wanted to transition to that form across the board. If you have all the info you need scanned already, you can do the form through Ebenefits and just attach the scanned docs to it that way and submit rather than dealing with snail mail, etc. You can always hand carry it to your RO later if you need to. Just print a copy from Ebenefits before you submit so you don't have to fill it all out again. VA doesn't accept physical discs/thumbdrives, etc, though, so youd need to print it all out if you hand carry/mail to the VA RO.

(I AM NOT A RATER- I work the claims BEFORE they are rated, annotating medical evidence in your records, VA and Legal documents,  and DA/DD forms- basically a paralegal/vso/etc except that I also evaluate your records based on Caluza and try to justify and schedule the exams that you go to based on whether or not your records have enough in them to warrant those)

having completed over a 1000 of these the last three years, I can repeat what brokensoldier stated. You can list as many disabilities on the 21-526ez as you can. If you run out of room, submit a VA FM 21-4138 with the remaining disabilities.

In your case you have 4. You will have no problem at all.

Ensure that you have all medical documentation to support each of your disabilities. If you submit additional documentation after submitting the FDC, it will take your claim out of the FDC status and make it a normal claim. Depending on your VARO, a normal claim can take upwards of 2 years. The shortest time I have seen for an FDC was 8 days and the longest was 8 months. Lot of variables to be fixed into the equation.
